⚠️ Artist: A previous reaction is your biggest red flag. Do not re-test a true cyanoacrylate allergy on the eye. Discuss a clear-adhesive plan, patch test, or decline the service.
⚠️ Artist: Serums often over-curl the natural lash and weaken the bond surface. Set retention expectations, or ask the client to pause use before their next set.

  • The information I've provided is accurate and complete, and I'll tell my artist if anything changes.
  • I understand eyelash extensions are applied with a cyanoacrylate-based adhesive, and that irritation or an allergic reaction is possible even without a previous history. I'll contact my artist right away if I experience swelling, itching, redness, or discomfort that doesn't settle within 24 hours.
  • I understand my artist will recommend a length, weight, and style based on the health of my natural lashes, and that my final set may differ from an inspiration photo in order to protect my natural lashes.
  • I've been told about aftercare (no heat, oil, or salt; wash daily; don't pick or pull; sleep on my back or side) and understand that retention depends on my aftercare, lifestyle, and natural lash cycle.
  • I understand that losing a few natural lashes daily is normal shedding, and that I should reach out within 48 hours of my appointment if I have premature shedding so it can be assessed.
  • I've read and agree to my artist's booking, deposit, cancellation, and late policies.
  • I release my artist from liability for reactions or outcomes resulting from inaccurate information on this form, or from not following aftercare instructions.
